Field Trip Forms

All high school students MUST submit the required forms to attend our field trips

Interest Form: Students MUST submit an interest form for each field trip they would like to attend

Attachment A: Students only need to complete once per academic year
Emergency Form: Students only need to complete once per academic year unless they need to make changes to contact information

UCLA Waiver: Students MUST submit a unique waiver for each field trip they would like to attend
